What information we collect and why
Physiotherapists are required by law and the Health and Care Professions Council (HCPC), our regulating body, to maintain full, clear, and accurate records for all our interactions with the clients we serve. For Neuro Rehab at Home to assess and treat our clients we must collect and store personal information. This includes information which identifies you as an individual, or personal data.
We may collect the following information:

· Telephone / mobile number
· Email address
· Information pertinent to fulfilling our services on an individual’s or organisation’s behalf, such as information about your health
· Records of consent
· Occasionally we may use videos and photos
What do we do with the information we collect?
We use the information you have given us to provide a safe and effective service to you. Here at Neuro Rehab at Home we have a strong interest in ensuring treatment is tailored to meet your needs, and that the advice and treatment offered is high quality and evidence based. We use the information you have provided to help us understand you and your condition so that we can collaboratively understand the problems you are experiencing and adjust goals and treatments accordingly.
We may occasionally send you promotional emails, or contact you so that we can improve the service we offer.
Who do we share the information with?
We will share your personal data with your knowledge and consent with other appropriate professionals if applicable and appropriate e.g. GP, other medical professionals, medical insurance companies, solicitors. We will share information with other professionals for safeguarding purposes if you disclose any risk to yourself or others and this may be without your consent.
Security and Storage of Information
We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect online.
Client related clinical notes are stored in a secure cloud-based system which is GDPR compliant.
Personal information is stored in a secure environment, protected by security controls and two factor authentication.
The storage of your information is in line with guidance from the Information Commissioner’s Office, HCPC and Chartered Society of Physiotherapy (CSP).
We will retain your personal data at the end of our treatment contractual agreement for a further 8 years. This data will be retained for the protection of you and the service and is required by professional liability insurance Terms and Conditions. The records shall be kept for at least 8 years following the last occasion on which treatment was given. After this retention period data will be securely deleted.
Where you have requested information only or contacted us for details of our services, and where this does not progress to assessment and or treatment and we do not have any contracts with you, we will retain your personal data for a period of 18 months. Where you make a complaint we will retain the data for 10 years. Where you or law enforcement agencies inform us about any active investigation or potential criminal prosecution, we will comply with legal requirements when retaining this data. The retaining of data is necessary where required for contractual, legal or regulatory purposes. Sometimes we may need to retain your data for longer, for example if we are defending ourselves in a legal dispute or as required by law or where evidence exists that a future dispute may occur.
How we use Cookies
